How to Authorize

Supporting Your Commitment to Care

We are here to support healthcare practitioners (HCPs) who want to see their patients get the most out of life. ZYUS is committed to helping HCPs leverage the power of cannabinoids to optimize their therapeutic benefits. We make it simple for HCPs to authorize ZYUS’ cannabinoids formulations connecting more patients with better care.

Authorizing HCPs are only required to complete Step 1 of the 3-part patient registration process.

Step 1

Complete Medical Document

A healthcare practitioner is required to complete a Medical Document on behalf of their patient.

Step 2

Complete Patient Application

A patient is required to register online or complete the downloadable Patient Application (PDF). If the patient resides in a hostel, shelter, hospice or similar institution, please ensure they use the downloadable application (PDF).

Step 3

Confirmation of Registration

Once the Medical Document and Patient Application are complete, the patient will receive an email that confirms their registration. This introductory email will provide instructions about how to log in and order their authorized formulations.

Please note, a Medical Document is required for new authorizations and patient renewals.
